At Center for Spine, Joint & Neuromuscular Rehabilitation, physicians and mid-level providers have training and experience in orthopedic, neurology, biomechanics, and pain management which makes the medical providers especially adept at managing complex pain problems with the goal of quality care for the patient.
Coordinating your Care
Patients are seen by appointment only. Center for Spine, Joint, & Neuromuscular Rehabilitation requires a referral to become a patient of our practice.

Monday- Friday 7:00 a.m. - 5:00 p.m.

New Patients at your first appointment
- Bring a list of all your current medications. It is important that we know the medications, dosages, and how often you take them.
- Bring copies of recent imaging reports, scans, and lab results with you. It is important that we have all the proper information to aid us in your care.
- Complete all of your new patient forms prior to coming for your appointment.
- Bring your medical insurance card and photo identification on all visits to our facility.
All routine calls and prescriptions must be handled during regular office hours.
